Application Instructions

When completing the “Application for Admission to Degree Program In Teacher Education” be sure to supply all information that is requested. Incomplete applications will not be accepted. Remember to complete the following:

  1. List the mailing address where you currently receive mail-include the street/box number and/or apartment number, city, state, and zip code. All components are needed to make sure the mail is not only delivered to you, but also done in a timely fashion. If your address changes after you have submitted the application, it is your responsibility to notify the Office of Student Services in Maxim Doucet Hall, Room 105 or call the office at (337) 482-6681.
  2. Obtain all the required signatures on the “Official Signatures” form. You, the student, must sign under the Student’s Statement, a faculty member other than your advisor must sign under Professional Endorsement, and your advisor must sign under Signature of Advisor.
  3. Obtain a current copy of your transcript in the Registrar’s Office in Martin Hall Room 171. Submit the transcript with the “Application for Admission to Degree Program In Teacher Education”. A printout of your transcripts from ULink is not acceptable.

NOTE: Only completed applications meeting the listed requirements will be accepted and reviewed by the Office of Student Services in Maxim Doucet Hall, Room 105.

If you meet all of the program requirements, the Office of Student Services will complete the “Admission to Upper Division” card. Once this card is processed, you will be allowed to take upper division (300-400 level) courses.
